If you were born in Quebec
- Original birth certificate (long form from Registre de l’état civil)
- Original school documents (MELS transcript of marks or TDG)

If you were born in Canada, outside of Quebec
- Original birth certificate
- Proof of Quebec residence (driver’s licence or Medicare card)
- Original school documents (diploma, relevé des apprentissages)

If you were born outside Canada
- Citizenship card or permanent residence card or IMM-1000 form and CSQ (Certificat de sélection du Québec)
- Proof of Quebec residence (driver’s licence or Medicare card) or three consecutive years of tax papers ( Revenu Québec, Federal, Municipal, School tax, House tax, etc.)
- Educational equivalencies or Comparative evaluation (MIDI)

DESCRIPTION OF THE PROGRAM 

Are you empathetic?  Patient?  Dedicated?

The Institutional and Home Care Assistance program (New Version) reflects current practices and prepares you with knowledge, skills and values necessary to provide or assist clients of all ages with tasks of daily living (bathing, personal grooming, eating, maintaining mobility, etc. )
The program courses focus on the home and health-related services required by individuals with physical disabilities, cognitive impairments and chronic and acute illnesses. It enables you to learn through a hands-on approach, facilitated by experienced health care professionals. Graduates may work in positions that bear various job titles (home care attendant or orderly).

PREREQUISITES: Secondary 3 credits math, language of instruction and second language

RECOGNITION OF ACQUIRED COMPETENCIES  (RAC)

If you have been working or have personal experience in this trade, you may be eligible to receive this diploma through the Recognition of Acquired Competencies (RAC) service.
